COMPARATIVE ANALYSIS OF THE IMPACT OF COMBINED APPLICATION OF OZONIZED PERFTORAN AND GLUTOXIM ON PROCESSES OF LIPID PEROXIDATION AND ENDOGENOUS INTOXICATION IN GENERALIZED PERITONITIS

Abstract


The paper analyzes the results of surgical treatment of 74 patients with generalized peritonitis. The severity of peritonitis was assessed by Mannheim peritoneal index (MPI). According to the method of treatment the patients were divided into two groups. A comparison group consisted of 38 patients who were treated in accordance with generally accepted principles. In 36 patients from the main group in the complex treatment we used local and systemic ozone therapy (OT) with ozonized perftoran (OP) and immunocorrection: all patients in the preoperative period were administered OP intravenously at a dose of 1 ml/kg with ozone concentration of 5 mg/l and 2 ml of a 1% solution of metabolic immunomodulator glutoxim. After surgery for systemic and intraperitoneal application of OP and glutoxim, the dose was adjusted according to severity by the MPI. In the blood serum before surgery, at 1st, 3rd, 5–7th and 10–14th days after surgery we determined indicators of lipid peroxidation — diene conjugates (DC), malondialdehyde (MDA), the activity of catalase (CAT), in serum and urine — the levels of peptids with medium molecular weight (MMDP), rest nitrogen, creatinine, urea, content of total protein, albumin, electrolytes — Na+, K+, Ca2+ and liver function indices — AST, ALT, AP, total bilirubin. Studies have shown that in both groups at admission to hospital, the depth of changes depended on GP severity by MPI: the greater the severity of peritonitis, the higher the content of DC, MMDP, total bilirubin, rest nitrogen, urea, creatinine, and conversely, lower total protein content, albumin, activity of CAT. Systemic and topical application of OP in combination with local and systemic immune correction with glutoxim in GP affects all pathogenetic components of the processes of lipid peroxidation, endogenous intoxication and thereby accelerates the overall detoxification of the body.
