Ultrasound Diagnosis of Diapeutics of Acute Pancreatitis

Abstract


Acute pancreatitis is one of the most common and severe acute surgical diseases of the abdominal cavity. One of the major problems is the choice of treatment strategy in various forms of acute pancreatitis, debated issues on the methods of conservative and surgical treatment are not resolved too. Modern trends in the treatment of patients with acute pancreatitis are increasingly oriented towards a preferred use of sparing minimally invasive interventions in the treatment of complications of pancreatic necrosis.
We have analyzed the results of diagnosis and treatment of 1350 patients. All patients underwent ultrasound investigation of hepatopancreatobiliary zone. Doppler ultrasound was used in combination with the main B-mode to detect the prevalence of localization of inflammatory and necrotic changes in the pancreas and parapancreatic zone. Minimally invasive surgery under ultrasound guidance were performed to 590 patients with the presence of complications of pancreatic necrosis. Duration of drainage standing was determined by clinical and sonographic data. After the cessation of discharge through drainage, the regression of the inflammatory infiltrate, disappearance of abnormal cavity we removed the drainage.
Minimally invasive surgery under ultrasound guidance is an effective surgical support in the treatment of such complications of pancreatic necrosis, as omentobursit, abscess of the pancreas or omental bursa, acute pancreatic pseudocysts, liquid collectors of the abdominal cavity and retroperitoneal phlegmons.
