Innovative Technologies in the Surgical Treatment of Hepatocellular Carcinoma with Portal Vein Tumor Thrombosis

Abstract


The objective of the study was to improve treatment outcomes in patients with hepatocellular carcinoma with tumor invasion of portal vein and inferior vena cava. The study included: 1st group — 22 patients with hepatocellular carcinoma with portal vein tumor invasion II–III stage by Shi, who underwent liver resection with portal vein resection and portoplasty; 2nd group — 16 patients with tumor invasion of the inferior vena cava, who underwent liver resection with resection of inferior vena cava and cavaplasty; control group — 62 patients with tumor invasion of portal vein of I stage by Shi, who underwent liver resection only. Postoperative mortality was 9.1; 12.5 and 8.1 % in the 1st, 2nd and control groups, respectively. An aggressive surgical approach in hepatocellular carcinoma with invasion of portal vein and inferior vena cava can increase long-term survival of patients.
