The short-term results of combined treatment of patients with squamous cell carcinoma of the head and neck using intra-arterial chemotherapy

Background. Malignant tumors of the head and neck are an important problem of modern oncology. Their proportion is about 10 % of all cancers. Despite the fact that many of the head and neck tumors could be visually detected, more than half of the cases are diagnosed at late stages, which dictates the need for multimodal treatments. Combined treatment with the use of intra-arterial chemotherapy is an effective and promising method for improving the results of treatment in this group of patients. The aim of the study was to evaluate the effectiveness of the combined treatment of patients with squamous cell carcinoma of the head and neck using intra-arterial chemotherapy. Materials and methods. 106 patients with morphologically confirmed squamous cell carcinoma of the oral cavity, oropharynx, hypopharynx and larynx without distant metastases have been enrolled in the investigtion. The study group included 52 patients, who underwent combined treatment with the use of intra-arterial chemotherapy and external beam radiotherapy. The control group consisted of 54 comparable patients, who were treated with combined systemic chemotherapy and radiotherapy. Results. In the study group after treatment, 30 (57.6 %) patients had complete, 12 (23.0 %) — a partial tumor response. The percentage of objective tumor response was 80.7 %. In the control group, complete resorption of the tumor was observed in 17 patients (31.4 %), partial — in 22 (40.7 %). The objective response rate was 72.2 %. Conclusons. Intra-arterial chemotherapy combined with radiation therapy in patients with squamous cell carcinoma of the head and neck is a safe and effective method of treatment, which has a relatively low rate of complications and can significantly improve the objective tumor response.
